- Physician education for proper hyperbaric fitness assessment and screening
- Physician Statement for hyperbaric prescription ready for approval and signature from your doctor
2. Hyperbaric FITNESS ASSESSMENT Report: $199.00
Professional review by a licensed hyperbaric provider to include a 1 page fitness assessment that includes:
- Your full hyperbaric Risk assessment report along with a review of your health screening questionnaire by on a licensed hyperbaric provider
- Providing you with an overall status to help you determine if you are ‘fit’ to undergo hyperbaric oxygen therapy, based on your questionnaire.
- A full one page summary statement signed off by a licensed hyperbaric provider
3. Hyperbaric PHYSICIAN CONSULTATION AND ASSESSMENT Report $349.00
Telemedicine hyperbaric consultation and a signed 1 page physician assessment statement includes:
- Your full hyperbaric risk assessment report along with a consultation review by a licensed hyperbaric provider
- Educate and answer questions regarding this procedure
- Hyperbaric protocol and dosages
- Signed physician statement – take to any hyperbaric center or for home hyperbaric therapy
